New Patients
All persons interested in becoming a patient at Van Zant Medical Group, S.C. must provide a concise summary of their medical concerns. Dr. Van Zant reviews each summary and, if appropriate, assigns each person to a provider in the practice. The patient will then be contacted by our office to schedule a new patient appointment.
All new patients must provide a $250 deposit to hold their appointment time when they are scheduled. This deposit will be credited toward their first office visit fee. To submit a new patient request please click the link below:

Important considerations

Our practice does not act as a primary care office, nor provide any traditional primary care services. Our patients are expected to maintain a separate primary care doctor for urgent needs and traditional doctor’s visits.

Our office does not bill any insurance. All visits are paid on the day of the appointment.

Each provider maintains her own panel of patients.

The initial visit at our office typically lasts 90 minutes; however if your medical concerns are complex, you may be scheduled for a 2 hour initial appointment.

Once you are scheduled for a visit, you will be sent the Office Policies and Fees document to review and sign within your patient portal.

You should receive a call regarding your new patient request within 2 to 4 weeks of submitting your medical summary.
